Annual Diabetic Eye Screening

If you are a Diabetic patient registered with the Practice, you will shortly receive an invitation letter with a Date and Time  to attend the Chet Valley Medical Practice for eye screening.

This letter will be from the Diabetes Eye Screening Service.

PLEASE make every effort to attend. If you are unable to attend, please refer to the letter for contact details of whom to inform.  The appointment can then be allocated to another patient.

Loddon Gym - Jubilee Hall Loddon - NR14 6NB

Loddon Community Gym is a well-equipped not-for-profit gym which provides sessions three times a week, whether it is for high performance workouts, gentle fitness or rehabilitation exercise. Sessions are held  Mondays - 1.30 - 6pm, Tuesdays - 4.30 - 8.30 pm and Fridays 10.00 - 1pm with the exception of Public holidays.  There is an annual membership of £18 and each session costs just £4.50.

 

Prescription Fees

From 1 April 2018 the prescription charge increased to £8.80 for each item or appliance dispensed.

 

Global Diagnostics NHS X-Ray Service

(Added 25 January 2018)

Patients for whom the GP has recommended an NHS x-ray will be given the option for referral to the Global Clinic, Watton Road, Norwich. The service will provide:- Rapid access to appointments - Monday - Friday 8.30 am - 8 pm, reports back to the GP usually within 48 hours and ample parking.  A GP referral is required. Patients will be allocated an appointment by Global.

 

Carers Matter Norfolk

Could you spare a couple of hours a week to befriend or support an unpaid carer – someone caring for a family member, friend or neighbour, who without their support, would be unable to cope? This could involve visiting a carer in their home or at a local café or community venue, providing listening support or helping them  to attend a local group or social activity.  If so, Carers Matter Norfolk, the new local publicly funded free support service for unpaid carers would like to hear from you. We can offer full training and support and will pay expenses to volunteers. All volunteer applications are subject to a satisfactory DBS check. GENERIC MEDICATION

(Added 1 December 2017)

Most medicines have two names - the unbranded (Generic) and the brand name that the company who first produced it may use.

As a result of imposed arrangements by our drug wholesaler we are no longer able to guarantee a specific make of generic medication.

Where appropriate we will continue to work within NHS budgets and prescribe medication by its Generic name but will not be able to guarantee that the medication is from the same manufacturer. The type and strength of your medication will remain the same.

SUMMARY CARE RECORD AND ENHANCED SUMMARY CARE RECORD

Many patients have a basic Summary Care Record which allows other services such as hospital A&E departments and Out of Hours to view basic details i.e. address, current medication and reactions to medicines.

We can upgrade a Summary Care Record by making it an Enhanced Summary Care Record with your consent.  This means allowing access to additional information such as:

  • Long Term Conditions
  • Relevant Medical History
  • Healthcare Needs and Personal Preferences
  • Immunisations
